Job Description:
H2 Health is seeking a dedicated and compassionate Pelvic Floor
Physical Therapist to join our dynamic team of healthcare
professionals. In this specialized role, you will focus on the
diagnosis, treatment, and management of pelvic floor dysfunctions,
providing expert care that enhances patients' quality of life. This
position involves collaborating within a multidisciplinary team to
ensure a tailored therapeutic approach that aligns with our
commitment to optimal, client-focused care, whether at our
state-of-the-art facilities or within the comfort of our patients'
homes. Evaluate and diagnose pelvic floor disorders using both
standard physical therapy assessments and specialized techniques.
Develop and implement individualized treatment plans based on
patient-specific needs, goals, and evidence-based practices.
Educate patients and their families about pelvic floor dysfunctions
and their management, promoting an understanding of therapy
benefits and expectations. Maintain accurate and timely
documentation of treatment plans, progress notes, and discharge
summaries. Collaborate with other healthcare providers to
coordinate treatment and provide holistic care. Participate in
ongoing education to remain current with the latest trends,
techniques, and research in pelvic floor therapy. Expert knowledge
and clinical skills in the assessment and treatment of pelvic floor
disorders. Strong interpersonal and communication skills, with an
emphasis on patient-centered care. Ability to design and deliver
comprehensive treatment plans. Proficient in the use of diagnostic
and therapy equipment pertinent to pelvic floor therapy. Medical

Valid Physical Therapy license in the state of practice.
Certification in Pelvic Floor Physical Therapy (CAPP-Pelvic) from
the American Physical Therapy Association (APTA) is preferred.
